All athletic paperwork will be completed online. This process will allow us to be more efficient in handling the forms, as well as save valuable resources. These forms are required each year your son or daughter wishes to participate in athletics for PISD. The Athletic Department would like to thank you and your son/daughter for filling out the forms and submitting them in a timely manner.
You must have your athlete’s school ID number in order to complete the forms. The forms can be found by going to www.rankonesport.com and clicking “fill out electronic forms” in the upper right hand corner. From there, follow the prompt and select “Pleasanton ISD”.
Once at Pleasanton’s home page, you will hover over the “electronic forms” tab, and select the form you wish to fill out. Enter in all of the information, sign by holding down your mouse while in the signature box, and click submit at the bottom of the page. You will be redirected to a page saying your form has been submitted for review. If you have not been redirected, you may have left a box blank. If something does not apply to your student, just type “NA” in the box. Repeat this process until you have completed both electronic forms. Do not place the zero at the beginning of your child’s ID number.
To support social distancing and assist with the workload of the medical professionals on the frontlines of the COVID-19 pandemic, the UIL Medical Advisory Committee (MAC) has recommended adjusting requirements related to preparticipation physical examinations for the 2020-21 school year.
THIS IS A NEW FORM – It MUST say 2020 in the upper right hand corner and contain the question on ECG testing to be accepted.
- All students participating in UIL marching band and/or UIL athletic activities will be required to complete the UIL Medical History Form. Any “yes” answer to any of questions 1-6 on the Medical History Form will require a student to receive further medical evaluation and acquire written clearance
- Any participant in athletics or marching band, in any grade level, who has not previously completed a Pre-Participation Physical Evaluation (PPE), and been cleared for participation, will be required to complete the Medical History Form as noted above, as well as a PPE prior to participation in any UIL practices, games, performances, or matches.
Medical history forms will be passed out May 4th, 5th, 6th, and 7th from 11-1 during equipment pickup, and distributed through google classroom and posted below:
DEADLINE TO TURN IN MEDICAL HISTORY FORMS IS MAY 15TH.
Forms will be collected at the Athletic office on Wednesday, May 13th, Thursday, May 14th, and Friday, May 15th from 10-12 and 3-5. They must be turned in to either Tab Dumont (Athletic Director) or Yvette Hotchkin (Athletic Secretary). Please be sure both parent and child sign the form.
